What is a Patient Centered Medical Home?

A Patient Centered Medical Home (PCMH) is a team based approach to providing and coordinating primary care services to all of our patients including children, youth and adults. Our PCMH team works in partnership with our patients, patients’ families, specialists and any other needed health care entities to ensure that our patients receive the most comprehensive care possible.

Our PCMH Responsibilities to our patients:

  • To listen and address your questions and medical concerns in a way that is easy to understand
  • To coordinate your overall care in our office and across other health care settings
  • To provide you with same day appointments when possible
  • To provide access to care at times that our office is closed
  • To clearly explain any medical disease, test results and treatment options so that you can make informed decisions about your health
  • To provide access to evidence based care and self-management support
    • Evidence based care: This means that your doctor will use the best available research combined with their experience and clinical expertise to make the most appropriate medical recommendations for you to follow.
    • Self-Management Support: This means that your doctor will explain all diseases in an easy to understand way so that you can clearly understand what steps you can take to positively affect your health. This will help you to make informed decisions and in turn to actively adopt healthy behaviors.
  • To provide language interpreters whenever possible to help us better communicate with our patients

Patient Responsibilities as part of a PCMH Team

  • To always provide us with your most up to date health history and any health changes that you have experienced
  • To always ask questions and be proactive in your care
  • To always call our office first with health concerns unless it is an emergency situation
  • To avoid using the emergency room for non-emergency medical issues
  • To always inform our office when you have received care at any other health care facilities or you have self-referred to a specialist so that we can obtain these records
