We are excited to share with you that we have renamed the Hallmark Health Medical Associates practice that you visit to Tufts Medical Center Community Care. This name change comes as part of the growing partnership between MelroseWakefield Hospital, Lawrence Memorial Hospital of Medford and Tufts Medical Center.
While the practice has a new name, the providers and staff whom you trust remain the same, continuing to provide you exceptional care. By partnering with Tufts Medical Center, our providers will be able to share best practices, ultimately offering you and your family more locations, more services and state-of-the-art technologies to help you live a healthy lifestyle.

Q. How will this benefit me as a patient?
A. By coming together, your providers will be able to share best practices and ultimately, we can offer you and your family more locations and provide more services and state-of-the-art technologies together to help you live a healthy lifestyle.
Q. Will I still be able to see my provider? Will there be other changes at my physician’s office?
A. Your provider and office staff aren’t changing. The providers and staff whom you trust as part of Hallmark Health Medical Associates will remain right there to care for you. As we make the transition, you may occasionally see both the Tufts Medical Center Community Care name and logo and the Hallmark Health Medical Associates (HHMA) name and logo at your provider’s office or elsewhere such as a bill or other paperwork. This will not cause any issues with payments, getting test results or anything else related to your care.
The name change will not affect how you connect with your provider. You will continue to contact your provider’s office using the contact information you use today. Our number one goal is to make the transition as seamlessly as possible.
Q. If I need to go to the hospital, will I now need to go to Tufts Medical Center in Boston?
A. No. Your practice remains part of MelroseWakefield Healthcare. If you require the services of a hospital, your provider will refer you to MelroseWakefield Hospital in Melrose or Lawrence Memorial Hospital in Medford. If you require additional or highly specialized services that aren’t available at MelroseWakefield Hospital or Lawrence Memorial Hospital, we are pleased to be collaborating with Tufts Medical Center and can seamlessly refer you to Tufts Medical Center in Boston.

Tufts Medical Center Community Care is a strong network of 21 practice locations and 93 providers located north of Boston. The network is affiliated with MelroseWakefield Hospital in Melrose and Lawrence Memorial Hospital of Medford for local care, and with Tufts Medical Center for highly specialized care not available locally.
Tufts Medical Center Community Care practices offer internal medicine, family medicine, OB/GYN, and multiple medical and surgical specialties. In addition, these practices partner with Tufts Medical Center Primary Care, which has five practice locations with 20+ providers south, west and north of Boston, as well as an academic medical center primary care practice in downtown Boston, enhancing access to primary care through eastern Massachusetts.
