Hallmark Health Medical Associates will soon change its name to Tufts Medical Center Community Care

MelroseWakefield Healthcare and Tufts Medical Center have been working together to provide exceptional clinical care close to your home. Recently, we’ve partnered to provide specialty services, including neurosurgery, advanced endoscopy, neonatal care, and cancer care.

We are excited to announce that we will now be partnering in primary care, family medicine, and even more medical and surgical specialty areas. To reflect our growing partnership, later this spring, our physician offices that are currently part of Hallmark Health Medical Associates will be renamed Tufts Medical Center Community Care.

About HHMA and Tufts MC Community Care

Hallmark Health Medical Associates is a strong network of 21 practice locations and 93 providers located north of Boston. The network is affiliated with MelroseWakefield Hospital in Melrose and Lawrence Memorial Hospital in Medford. These practices offer internal medicine, family medicine, OBGYN, and medical and surgical specialties.

Tufts Medical Center Community Care (Tufts MC CC) is Tufts Medical Center’s community-based primary care employment organization that provides Eastern Massachusetts communities with access to top-tier primary care services. Tufts MC CC has four practice locations and 21 providers south, west and north of Boston. In addition, these practices partner with the academic primary care practice of Tufts Medical Center in Boston.

Answering Your Questions

Q. When is this happening?
A. We will be transitioning the Hallmark Health Medical Associates’ practices to the Tufts Medical Center Community Care name in the spring of this year. We will keep you well informed along the way.

Q. How will this benefit me as a patient?
A. By coming together, your providers will be able to share best practices and ultimately, we can offer you and your family more locations and provide more services and state-of-the-art technologies together to help you live a healthy lifestyle.

Q. Will I still be able to see my provider? Will there be other changes at my physician’s office?
A. Your provider and office staff aren’t changing. The providers and staff whom you trust as part of Hallmark Health Medical Associates will remain right there to care for you. As we make the transition, you may occasionally see both the Tufts Medical Center Community Care name and logo and the Hallmark Health Medical Associates (HHMA) name and logo at your provider’s office or elsewhere such as a bill or other paperwork. This will not cause any issues with payments, getting test results or anything else related to your care. The name change will not affect how you connect with your provider. You will continue to contact your provider’s office using the contact information you use today. Our number one goal is to make the transition as
seamlessly as possible.

Q. If I need to go to the hospital, will I now need to go to Tufts Medical Center in Boston?
A. No. Your practice remains part of MelroseWakefield Healthcare. If you require the services of a hospital, your provider will refer you to MelroseWakefield Hospital in Melrose or Lawrence Memorial Hospital in Medford. If you require additional or highly specialized services that aren’t available at MelroseWakefield Hospital or Lawrence Memorial Hospital, we are pleased to be collaborating with Tufts Medical Center and can seamlessly refer you to Tufts Medical Center in Boston.
